Monthly TBM audit results. I audited twelve toolbox talks unannounced over the past month, across four subcontractors. Scores out of 100. Overall average: 72 — that's below our 80 target. Let me show the breakdown. Interaction score — are workers actually participating or is the supervisor just lecturing? Average: 6.2 out of 10. Specificity score — is the talk about today's specific task, or generic "work safely"? Average: 5.8. Documentation score — attendance sheets signed, topics recorded — average: 9.1. The forms are getting filled, but the content isn't hitting the mark.

Two things. First: supervisors are reading from generic scripts instead of doing a real-time hazard walk before the talk. They're saying "watch your footing" when the real risk is an unguarded leading edge at 8 meters on a roof they haven't even looked at that morning. The talk must be preceded by a physical walk of the work area. Second: zero worker participation. The supervisor speaks for 10 minutes, nobody asks a question because nobody was asked. Starting next week, I'm implementing three changes: (1) every toolbox talk must include at least one worker demonstrating a task or flagging a hazard — flip the classroom. (2) supervisors must walk the work area within 30 minutes before the talk and note three specific hazards — if they can't name three, they haven't looked. (3) HSE officers will audit two random toolbox talks per week using the new scoring checklist — and the scores will be posted on the site notice board. Public accountability.
