I've raised NCR-042 for the misaligned anchor bolts on the compressor foundation. Six out of twenty bolts are off by more than 15 millimeters.

That's a significant deviation. Have you issued the containment action? We need to stop any further concrete work until we figure out the fix.

Already actioned. We held a quality stand-down after the third repeat. Root cause was inconsistent welder qualification tracking across subcontractors. We've now centralized the welder database and added a pre-job verification step. No repeat welding NCRs in Q2 so far.
